Trauma-Informed Stabilisation Treatment

Trauma-Informed Stabilisation Treatment (TIST) was designed and developed by Dr Janina Fisher to support clients who have experienced prolonged trauma, particularly in early life.

Many clients find themselves stuck in repetitive cycles of Complex PTSD, self-harm, addiction, and identity confusion,
also as a result of traumatic attachment in early life and, in adulthood, difficulties in relationships with abusive, neglectful, or emotionally unavailable partners
. As a result, they often struggle with overwhelming emotions, persistent anxiety
or sadness, low self-worth, self-criticism, and chronic shame or guilt, among other difficulties
.
 
TIST helps by reframing and managing these repetitive behaviours as ways a client learned to survive, rather than as failures. By viewing symptoms as protective responses to trauma, the approach reduces shame
and self-blame, 
and supports a more compassionate relationship with what is happening internally.
 
The model offers a clear and practical neurobiologically grounded path to healing. Clients are supported in recognising triggers
and signs as coming from different parts
 of themselves that have 
learned to cope in overwhelming situations. 
Over time, TIST helps clients relate to these parts with curiosity and compassion, rather than fear, judgment, or shame.

TIST is uniquely suited for Clients who experience:

Emotional dysregulation

  • Feeling overwhelmed by emotions that come on quickly or intensely
  • Emotions that shift rapidly or feel hard to predict
  • Difficulty calming yourself once you’re upset
  • Feeling “too much” or, at times, emotionally shut down
  • Reacting strongly to situations that seem small afterwards

Chronic or Ongoing shame

  • A strong inner critic or feeling “not good enough”
  • A sense that something is wrong with you at your core
  • Fear of being judged, rejected, or exposed
  • Difficulty accepting care, kindness, or support
  • Pushing yourself to be perfect or to please others

Complex or Ongoing Trauma

  • Feeling overwhelmed by emotions or shutting down completely
  • Always being on alert, tense, or anxious
  • Feeling unsafe even when nothing is “wrong”
  • Difficulty trusting others or feeling close
  • Feeling exhausted from constantly managing your inner world

Relational difficulties

  • Being drawn to relationships that feel familiar but painful
  • Struggling with closeness or fearing abandonment
  • Feeling stuck in push–pull dynamics in closeness or intimacy
  • Repeating unhealthy relationship patterns, like being with unavailable or abusive partners
  • Difficulty naming or asking for your needs 

Self-sabotaging

  • Undermining your own success or happiness
  • Procrastinating or avoiding tasks that could improve life circumstances
  • Making impulsive decisions that undermine long-term goals
  • Pushing people away or creating conflict, even when you want a connection
  • Feeling stuck in cycles you want to break, but struggle to change

Attachment Loyalties

  • Putting others’ needs before your own, even when it hurts you
  • Maintaining familiarity with one's own safety
  • Setting boundaries feels scary, wrong, or selfish
  • Feeling responsible for other people’s emotions
